In 2008, Dr. Daniel A. Anzaldua founded Hamilton Nephrology, LLC – now Kidney Professionals of Indiana – to offer patients with kidney disease a better alternative and a more personal approach. As an expert in Nephrology, Dr. Anzaldua creates customized treatment plans for each individual patient and uses advanced approaches for care, resulting in better outcomes. Kidney Professionals of Indiana is also proud to partner with NxStage Kidney Care, the leader in home dialysis technology. Kidney Professionals of Indiana’s patients are rewarded from an individualized and personalized collaboration, which results in remarkable improvements in lifestyle.
In 2012, Dr. Anzaldua created Hamilton Medical Consultants, LLC, broadening the range of patients he treats. He now employs a team of providers to focus on post-acute patients, this translates to a reduction in hospital re-admissions, increases health care savings, and an overall improvement in health for patients across the care continuum.
Daniel A. Anzaldua
Looking beyond the diagnoses is at the core of Dr. Daniel Anzaldua’s patient care philosophy. Dr. Anzaldua collaborates with each patient on a deeper level. He seeks to understand first and treat second. As a physician, Dr. Anzaldua values interacting with his patients, providing the highest level of service to each person in his care by formulating comprehensive treatment plans that lead to optimal outcomes in the context of their entire well-being.
Dr. Anzaldua holds degrees from St. Mary’s University and Saba University School of Medicine. He completed his Internal Medicine Residency at St. Vincent Hospital in Indianapolis and his General and Interventional Nephrology Fellowship at Saint Louis University. Following his training, he provided specialized locum tenens nephrology services to rural and veteran’s hospitals across the country. He is board certified in Internal Medicine and Nephrology. In addition to his private practice, Dr. Anzaldua currently serves as Medical Director at Indiana Veterans’ Home in West Lafayette, Indiana and Evergreen Crossing & The Lofts, Green House Cottages of Carmel, and Lynhurst Healthcare in Indianapolis. He has also started a program to treat veterans at the Indiana Veterans’ Home with sensory medicine techniques.
Born and raised in Texas, Dr. Anzaldua has lived in Carmel, Indiana since 2003. Though Dr. Anzaldua’s life-long love of learning led him to his passion for medicine, he enjoys a well-rounded, balanced life. He loves the outdoors, writing, soccer, golf, card games, and traveling.
Jennifer A. Arnett
Jennifer Arnett has more than 15 years of experience as a Nurse Practitioner. Her focus has been on working with patients in post-acute and long-term care settings. She specializes in chronic illness, diabetes, and endocrine disorders, infectious disease, kidney disease, and acute and rehabilitative care. Ms. Arnett joined Kidney Professionals of Indiana because of her passion to provide a greater level of care to patients with kidney-related disorders, exemplified by the leadership of Dr. Anzaldua.
Originally from South Bend, Ms. Arnett holds a nursing degree from Indiana University and her Masters of Science in Nursing and Adult Nurse Practitioner degrees from IUPUI. She is a member of the American Academy of Nurse Practitioners and the American Medical Directors Association (AMDA). Outside of work, Ms. Arnett enjoys spending time with her two children, as well as reading and watching sports – especially IU basketball.
Tara S. Lutes
Manager of Operations/General Counsel
Tara Lutes assists with the legal, operational, financial and strategic missions of Kidney Professionals of Indiana. She values being part of a small business practice while navigating the ever-changing landscape of the healthcare market. She is proud to be part of a team of hardworking and forward-thinking individuals who are changing the way Nephrology patients receive care.
Ms. Lutes grew up in Lafayette, Indiana. She holds her undergraduate and law degrees from Indiana University. Prior to joining Kidney Professionals of Indiana in 2014, she worked nearly 15 years in private practice law, concentrating on insurance defense coverage and litigation. Outside of work, Ms. Lutes spends time with her husband, two young children, and dog. Beyond the activities of an active mom, she enjoys traveling, visiting friends and watching movies. She also has a passion for reading books, reading design blogs and practicing yoga.
Karen M. Kress
Karen Kress values witnessing the impact Kidney Professionals of Indiana has on every patient who walks through the door. She is the main point of contact and the one who assists with the tasks of paperwork and billing so each patient can focus on what truly matters – their care. Ms. Kress strives to make communication with patients her top priority and is proud to partner with the clinical team members to create individual patient care experiences.
Born and raised in Indianapolis, Ms. Kress holds a degree in English and Theatre Arts from Indiana University. She worked for several years in the non-profit arts sector before joining Kidney Professionals of Indiana in 2011. Outside of work, Ms. Kress enjoys spending time with her husband and five children, running, playing tennis, and actively volunteering in her community.
Renee A. Schwegman
Renee Schwegman finds great joy in her hands-on interaction with patients during their visits to Kidney Professionals of Indiana. Ms. Schwegman loves meeting new people and cultivating meaningful patient/practice relationships. As a medical assistant, she ensures each patient is comfortable and treated with the best care possible. She joined Kidney Professionals of Indiana in 2016, where she values working with a great team to meet the needs of every patient.
Born and raised in Batesville, Ms. Schwegman holds a degree in psychology from Indiana University. She loves spending quality time with her family, attending concerts, working out and watching sports of all kinds.
Nicole M. Stephenson
Nicole Stephenson takes pride in providing compassionate care for each patient at Kidney Professionals of Indiana. As a medical assistant since 2013, she enjoys interacting with patients and creating a smooth administrative journey for each patient. Her goal is to promote a transparent communication policy with every patient in KLidney Professionals of Indiana’s care.
Ms. Stephenson grew up in Indianapolis and attended IUPUI. Outside of the office, she enjoys spending time with her husband, two sons, and daughter, as well as going to movies, trying new restaurants and attending professional sporting events.
Alice M. Hampton
With more than 16 years in healthcare, Alice Hampton joined the team at Kidney Professionals of Indiana in 2017. Believing in a patient experience that is comforting and warm, she extends kindness to each and every patient. From taking a patient’s vitals to scheduling their next appointment, each interaction is met with a smile. This is her way of contributing to the comprehensive care delivered by our team.
Ms. Hampton grew up in Anderson, Indiana. She holds an associate of science degree in Health Sciences. Outside of work, Ms. Hampton enjoys singing, traveling and fishing with her father, in addition to spending quality time with her family.
